We offer two options for completing our office paperwork.

To minimize time in the waiting room we offer some of our initial intake paperwork via download from our web site. Click on the below files to open another window or right click on the below links and select "save target as".

Please fax in the completed forms, along with copies of current lab work to (802) 318-4467.
- If a fax is unavailable; download, fill out and bring the completed forms with you to your office appointment.

What you should expect during the first visit:
The day of your first appointment, Dr. Wdowin will take an extensive history, review lab work, perform any necessary physical exams, order any necessary laboratory tests, and answer questions you may have. Office visit fees do not include any laboratory fees, treatments, supplements, or costs other than the office visit itself.
Dr. Wdowin does not have an in-house laboratory. Therefore, all necessary blood work will be ordered by Dr. Wdowin and performed at a Sonora Quest draw station.
Subsequent follow-up visits with Dr. Wdowin will occur as he deems necessary.
Is Dr Wdowin my primary care physician?
All patients are instructed to have a primary care physician as Dr. Wdowin is not affiliated with a hospital and will not be performing yearly physicals.
Insurance information?
Certain insurance carriers provide reimbursement for Naturopathic medical care and others don’t. Due to the complexity and sheer number of different insurance plans and insurance companies we do not accept insurance in our office and inform all of our patients that they are responsible for all financial costs at the time of service. We will however provide you will the necessary form to file on your own behalf so your insurance company reimburses you directly.
